In men with more advanced degrees of hair loss, one popular option used on this 50yo is to “frame” the face by creating a frontal forelock that connects with the sides.

Patient presented several years after multiple failed attempts to remove prior artificial hairs implants (Biofibres) that were chronically infected. Dr. Epstein performed a wide excision of the scalp, essentially a modified scalp reduction, allowing for the removal of more than 90% of the affected scalp.

This 29 year old African American patient underwent a surgical hairline advancement (SHA) that lowered his hairline more than 35 mm, or 1 ½ inches. For a man to be a good candidate for this surgery, the scalp needs to be flexible, and the hairline stable. The SHA provides a denser hairline than what can typically be achieved with hair grafting.

Unusual reparative hair case, on a patient with prior grafts by another surgeon, which left an unnatural appearance. Another surgeon had attempted to do FUE punch removal of the prior grafts, leaving additional scarring and not improving the appearance. Note that FUE punch removal can be a very effective procedure, but in this case, the patient was better treated with a bilateral brow lift-like excision of not only the prior grafts but also the scarred skin, healing as a fine line scar.
Highly specialized reparative hair procedure of work done elsewhere called a surgical hairline excision, done in this case to remove the frontal 2 inches of hairline that were scarred and pluggy in appearance, then closed back as a fine line scar. This is a one-hour long procedure that is reserved for those cases of significant scarring of the skin as well as for a pluggy appearance

This patient presented 30 years having had plug grafts placed too low and unaesthetically. Because of the volume of plug grafts and the scarring of the skin along the hairline, the decision was made to do a complete hairline excision, removing all of the plugs and the surrounding scarred skin, resulting in a much softer and higher hairline. Note that the plug graft hairs that were excised were dissected down then planted into the midscalp region but these hairs are just starting to regrow. Patient returned for grafting at 5 months to help conceal the scar and create better density.
