Hair Transplantation Using the FUE Approach

The state-of-the-art technique of FUE (follicular unit extraction) hair transplantation permits, when performed aesthetically by an experienced surgeon, the growing of hairs in areas of the scalp where there is thinning or complete absence whether due to loss or never having grown there due to genetics. This is a refined procedure that requires several steps, starting with the design of the restoration, followed by the extraction/harvesting of the grafts one-at-a-time, then the making of the recipient sites in the areas where more hair is to be placed, and finally the placement of the hair grafts into these recipient sites.

The grafts can be placed into areas of thinning such as the frontal or crown region, or where there is little to no hair such as the frontotemporal recessions or the entire hairline or sideburns or eyebrows. Prior to transplantation in areas of thinning, to assure optimal results most patients are first placed on medical therapy to stabilize the hair loss. Grafts- each containing from one to three hairs- are then harvested from the scalp using the FUE approach, from areas of the scalp most commonly the back of the head that are chosen for their ideal curl, texture, and color as well as being typically “permanent” so they will continue to grow for a lifetime. Most women have the grafts extracted using a no-shave FUE approach so there is no detectability that anything was done to the scalp.

Making of the recipient sites is performed using the smallest possible blades typically 0.5 to 0.7mm in size. It is here that Dr. Epstein’s expertise and aesthetic skills allow these recipient sites to be made at exactly the desired angle, direction, and position to achieve truly natural outcomes. These recipient sites can number in the hundreds to the thousands, with it not uncommon to have 2000 or more grafts (equivalent to 4000 or more hairs) in a single procedure. Once made, the prior extracted grafts are placed into the recipient sites using implanters, assuring the grafts follow the angles and directions as designed.

These are truly painless procedures. Most patients choose to take oral sedation as well as laughing gas/ProNox®, but also offered is twilight intravenous sedation. There is never any trimming of the hairs in the recipient areas where the new hairs are placed, thus most patients can conceal the work done and return to regular activities a day or two later, with resumption of full exercise on day six. The transplanted hairs typically fall out in a week or two, then start to regrow at three to four months and are permanent.

Hairline Lowering & Forehead Reduction

There are two techniques for advancing the overly high hairline – forehead reduction/hairline lowering surgery, and hair transplantation. Few doctors are proficient in both procedures, but given Dr. Epstein’s experience as a facial plastic surgeon specializing exclusively in hair restoration, he is one of the world’s specialists.

The surgery takes two hours to perform, and allows the appropriate candidate to achieve as much as 1 inch or more of lowering of the hairline in a single procedure, with virtually instantaneous results and unsurpassed density. The main requirement for this procedure is a flexible scalp to allow for sufficient advancement, and not much if any thinning of the frontal hairline. Hair grafting is less “surgical”, where 1200 to as many as 2500 or more hair grafts can be transplanted in a single procedure to grow in a natural direction. Surgical Hairline Advancement/Hairline Lowering Surgery

This surgical procedure, in which Dr. Epstein is one of a handful of surgeons to specialize performing, involves the making of an incision along the front of the hairline, through which the entire frontal scalp is brought forward and the excess forehead skin excised. The incision heals as a fine line scar designed so that hairs actually grow through it. A unique modification developed by Dr. Epstein permits rounding out of the hairline to achieve the most feminine-appearing outcome.

Usually performed under general anesthesia, recovery is quite easy, with sutures removed at eight days and most patients presentable the second day. In most cases, Dr. Epstein is able to advance the hairline by 20 to 24mm, or around one inch, but he routinely exceeds this amount due to his extensive experience that allows him to obtain the greatest amount of lowering. Dr. Epstein is also able to use this surgery to repair prior transplanted hairlines that lack density and or have developed scarring due to less than aesthetic work. Dr. Epstein may also use this technique in cases of scalp reconstructions such as after trauma. The surgery can also be combined if desired with browlifting and/or reduction of the prominence of the forehead/frontal bone.

Hair Grafting

This meticulous technique takes five to as long as 10 hours to perform, usually under oral sedation, to allow for the harvesting then placement of the large number of hairs into individually made recipient sites.

The hairs are transplanted in what are called “follicular units” — the natural bundling of one, two, and three hairs. No sutures are required nor is there any scarring because the procedure is done using the FUE (follicular unit extraction) technique. The next day the patient may have her hair washed, then return to most activities in a matter of days.
